Two transportation disasters highlight how a combination of structural weakness and human factors can push routine journeys into life-threatening emergencies—and ultimately reshape safety standards.
In one case, a passenger aircraft experienced a sudden and violent decompression mid-flight when a large section of its fuselage tore away. Despite the extreme conditions, the crew managed to maintain control and land the aircraft, saving many lives.
