Sometimes documentation is necessary. But documentation is not magic. It records the decision. It does not rescue it.
This episode explains why documentation cannot fix unclear scope, weak pricing assumptions, unpriced travel, material risk, unsupported ODCs, informal customer direction, subcontract misalignment, or fixed-price commitments the company should not have accepted in the first place.
As fixed-price and performance-based contracting pressure increases, contractors need to focus less on decorating weak decisions after the fact and more on making better decisions before risk is accepted.
