For the first time in history, the world shut itself down - by choice - all for fear of a virus, COVID-19, that wasn’t well understood. The government, with the support of most Americans, ordered the closure of tens of thousands of small businesses - many never to return. Almost every school and college in the country sent its students home to finish the school year in front of a computer. Churches cancelled worship services. “Social distancing” went from a non-word to a moral obligation overnight.

Technological innovation is deeply woven into the fabric of American culture, and is no less a basic feature of American health care. Medical technology saves lives and relieves suffering, and is enormously popular with the public, profitable for doctors, and a source of great wealth for industry. Yet its costs are rising at a dangerously unsustainable rate. The control of technology costs poses a terrible ethical and policy dilemma. How can we deny people what they may need to live and flourish?

Antibiotics are powerful drugs that can prevent and treat infections, but they are becoming less effective as a result of drug resistance. Resistance develops because the bacteria that antibiotics target can evolve ways to defend themselves against these drugs. When antibiotics fail, there is very little else to prevent an infection from spreading. The media constantly bombard us with news of health hazards lurking in our everyday lives. But many of these hazards turn out to have been greatly overblown. According to author and epidemiologist Geoffrey C. Kabat, this hyping of low-level environmental hazards leads to needless anxiety and confusion on the part of the public as to which exposures have important effects on health and which are likely to have minimal or no effect.

Our health care system is crippled by desperate efforts to prevent the inevitable. A third of the national Medicare budget - nearly $175 billion - is spent on the final year of life, and a third of that amount on the final month, often on expensive (and futile) treatments. Such efforts betray a fundamental flaw in how we think about healthcare: we squander resources on hopeless situations, instead of using them to actually improve health.
