Tammy’s internet boyfriend finally arrives, bringing "juicy lips" and zero intention of helping push a wheelchair up a slight incline. Tammy’s new doctor delivers a harsh reality: after gaining 50 pounds in one month, she must now lose 100 pounds to qualify for surgery. While Chris and Amy push for accountability, Tammy remains emotionally detached and resistant to change.

As Amy’s pregnancy advances, the family grapples with the fear that Tammy is giving up—and that Amy won’t be able to care for her sister once the baby arrives. The episode ends with growing distance as Amy and Michael consider moving, forcing Tammy to confront the possibility of being on her own.

Amy, meanwhile, decides the best way to support this fresh start is by doing a pregnancy cravings food challenge the night before seeing the doctor. Junior Mints on fruit, peppermint patties in egg rolls, pineapple lasagna, and sour gummies for the baby that now apparently runs her entire personality.

The Atlanta road trip delivers fart trauma, a mysterious heavy bag on the roof, Chris nearly passing out trying to push Tammy through a farm, and Amy almost getting taken out during fowling.
