In 2016, a United Nations report found the UK government culpable for ‘grave and systematic violations’ of disabled people’s rights. Since then, driven by the Tory government’s obsessive drive to slash public spending whilst scapegoating the most disadvantaged in society, the situation for disabled people in Britain has continued to deteriorate. Punitive welfare regimes, the removal of essential support and services and an ideological regime that seeks to deny disability has resulted in a situation described by the UN as a ‘human catastrophe’.

On a cold March day in 1893, 26-year-old nurse Lillian Wald rushed through the poverty-stricken streets of New York’s Lower East Side to a squalid bedroom where a young mother lay dying -abandoned by her doctor because she could not pay his fee. The misery in the room and the walk to reach it inspired Wald to establish Henry Street Settlement, which would become one of the most influential social welfare organizations in American history.

Growing up poor, Deborah Dundas knew what it meant to want, to be hungry, and to long for social and economic dignity; she understood the crushing weight of having nothing much expected of you. But even after overcoming many of the usual barriers faced by lower- and working-class people, she still felt anxious about her place, and even in relatively safe spaces reluctant to broach the subject of class. While new social movements have generated open conversation about gender and racism, discussions of class rarely include the voices of those most deeply affected: the working class and poor.

The sometimes fragmentary tales of these people, their lives and deaths, are not included in official histories of a place. How, Ensign asks, has a large, socially progressive city like Seattle responded to the health needs of people marginalized by poverty, mental illness, addiction, racial/ethnic/sexual identities, and homelessness? Drawing on interviews and extensive research, Ensign shares a diversity of voices within contemporary health care and public policy debates.
